What are diuretics and how do they work?
The amount of fluid (water) retained by the body is controlled primarily by the kidneys. This occurs due to the kidney's ability to control the retention and elimination of sodium and chloride, because the amounts of sodium, chloride, and water in the body are carefully balanced. Thus, if sodium and chloride are eliminated from the body, water also is eliminated. Conversely, if sodium and chloride are retained by the body, so is water.
The elimination of sodium, chloride, and water from the body is somewhat complex. In the kidneys, sodium, chloride, and other small molecules are filtered out of the blood and into the tubules of the kidney where urine is formed. Most of the sodium, chloride, and water are reabsorbed into the blood before the filtered fluid leaves the kidney in the form of urine. Aldactone (spironolactone) is an aldosterone receptor antagonist that causes the kidneys to remove water and sodium from the body, with reduced losses of potassium. Aldactone is used to reduce edema caused by heart, liver or kidney problems, hypertension, and certain patients with hyperaldosteronism. Aldactone is available in a generic form named spironolactone. Common side effects of Aldactone include skin rash, headache, dizziness, and GI symptoms of nausea, vomiting, gas, and stomach pain.
Aldactone is available in 25, 50 and 100 mg tablets. Serious side effects of Aldactone include hyperkalemia, altered heart beats, confusion, tremors, decreased or no urine output, shallow breathing, muscle pain or weakness, and numbness. Because of tumor formation in experimental animals, use in pregnancy should be avoided unless the benefits outweigh the potential risks to the fetus; women who are breastfeeding are advised not to use Aldactone. In addition, the drug should not be used to decrease the normal edema of pregnancy. Aldactone has not been proved safe or effective in the pediatric population.

Establishing the diagnosis of primary hyperaldosteronism by therapeutic trial.
Short-term preoperative treatment of patients with primary hyperaldosteronism.
Long-term maintenance therapy for patients with discrete aldosterone-producing adrenal adenomas who are judged to be poor operative risks or who decline surgery.
Long-term maintenance therapy for patients with bilateral micro or macronodular adrenal hyperplasia (idiopathic hyperaldosteronism).

Aldactone Consumer Information: Uses
Spironolactone is used to treat high blood pressure. Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. It is also used to treat swelling (edema) caused by certain conditions (e.g., congestive heart failure) by removing excess fluid and improving symptoms such as breathing problems.
This medication is also used to treat low potassium levels and conditions in which the body is making too much of a natural chemical (aldosterone).
Spironolactone is known as a "water pill" (potassium-sparing diuretic).
