Human chorionic gonadotropin (HCG), a polypeptide hormone produced by the human placenta, is composed of an alpha and a beta sub-unit. The alpha sub-unit is essentially identical to the alpha sub-units of the human pituitary gonadotropins, luteinizing hormone (LH) and follicle-stimulating hormone (FSH), as well as to the alpha sub-unit of human thyroid-stimulating hormone (TSH). The beta sub-units of these hormones differ in amino acid sequence.

PREGNYL® (chorionic gonadotropin for injection, USP) is a highly purified pyrogen-free preparation obtained from the urine of pregnant females. It is standardized by a biological assay procedure. It is available for intramuscular injection in multiple dose vials containing 10,000 USP Units of sterile dried powder with 5 mg monobasic sodium phosphate and 4.4 mg dibasic sodium phosphate. If required, pH is adjusted with sodium hydroxide and/or phosphoric acid. Each package also contains a 10 mL vial of solvent containing: water for injection with 0.56% sodium chloride and 0.9% BENZYL ALCOHOL, WHICH IS NOT FOR USE IN NEWBORNS. If required, pH is adjusted with sodium hydroxide and/or hydrochloric acid.

HCG HAS NOT BEEN DEMONSTRATED TO BE EFFECTIVE ADJUNCTIVE THERAPY IN THE TREATMENT OF OBESITY. THERE IS NO SUBSTANTIAL EVIDENCE THAT IT INCREASES WEIGHT LOSS BEYOND THAT RESULTING FROM CALORIC RESTRICTION, THAT IT CAUSES A MORE ATTRACTIVE OR “NORMAL” DISTRIBUTION OF FAT, OR THAT IT DECREASES THE HUNGER AND DISCOMFORT ASSOCIATED WITH CALORIE-RESTRICTED DIETS.

1. Prepubertal cryptorchidism not due to anatomical obstruction. In general, HCG is thought to induce testicular descent in situations when descent would have occurred at puberty. HCG thus may help predict whether or not orchiopexy will be needed in the future. Although, in some cases, descent following HCG administration is permanent, in most cases, the response is temporary. Therapy is usually instituted in children between the ages of 4 and 9.
2. Selected cases of hypogonadotropic hypogonadism (hypogonadism secondary to a pituitary deficiency) in males.
3. Induction of ovulation and pregnancy in the anovulatory, infertile woman in whom the cause of anovulation is secondary and not due to primary ovarian failure, and who has been appropriately pretreated with human menotropins.

For intramuscular use only. The dosage regimen employed in any particular case will depend upon the indication for the use, the age and weight of the patient, and the physician's preference. The following regimens have been advocated by various authorities:

Prepubertal cryptorchidism not due to anatomical obstruction. Therapy is usually instituted in children between the ages of 4 and 9.

1. 4,000 USP Units three times weekly for three weeks.
2. 5,000 USP Units every second day for four injections.
3. 15 injections for 500 to 1,000 USP Units over a period of six weeks.
4. 500 USP Units three times weekly for four to six weeks. If this course of treatment is not successful, another series is begun one month later, giving 1,000 USP Units per injection.

Selected cases of hypogonadotropic hypogonadism in males.

1. 500 to 1,000 USP Units three times a week for three weeks, followed by the same dose twice a week for three weeks.
2. 4,000 USP Units three times weekly for six to nine months, following which the dosage may be reduced to 2,000 USP Units three times weekly for an additional three months.

Induction of ovulation and pregnancy in the anovulatory infertile woman in whom the cause of anovulation is secondary and not due to primary ovarian failure and who has been appropriately pretreated with human menotropins. (See prescribing information for menotropins for dosage and administration for that drug product.)

5,000 to 10,000 USP Units one day following the last dose of menotropins. (A dosage of 10,000 USP Units is recommended in the labeling for menotropins.)

Directions for Reconstitution

Two-vial package: Withdraw sterile air from lyophilized vial and inject into diluent vial. Remove 1-10 mL from diluent and add to lyophilized vial; agitate gently until powder is completely dissolved in solution. Parenteral drug products should be inspected visually for particulate matter and discoloration prior to administration, whenever solution and container permit.

IMPORTANT: USE COMPLETELY AFTER RECONSTITUTION. RECONSTITUTED SOLUTION IS STABLE FOR 60 DAYS WHEN REFRIGERATED.

Two-vial package containing:

1-10 mL lyophilized multiple dose vial containing: 10,000 USP Units chorionic gonadotropin per vial, NDC 0052-0315-10.

1-10 mL vial of solvent containing: water for injection with sodium chloride 0.56% and benzyl alcohol 0.9%, NDC 0052-0325-10.

HCG should be used in conjunction with human menopausal gonadotropins only by physicians experienced with infertility problems who are familiar with the criteria for patient selection, contraindications, warnings, precautions, and adverse reactions described in the package insert for menotropins.

The principal serious adverse reactions during this use are: (1) Ovarian hyperstimulation, a syndrome of sudden ovarian enlargement, ascites with or without pain, and/or pleural effusion, (2) Rupture of ovarian cysts with resultant hemoperitoneum, (3) Multiple births, and (4) Arterial thromboembolism.

General

Since androgens may cause fluid retention, HCG should be used with caution in patients with cardiac or renal disease, epilepsy, migraine, or asthma.

Pediatric Use

Induction of androgen secretion by HCG may induce precocious puberty in pediatric patients treated for cryptorchidism. Therapy should be discontinued if signs of precocious puberty occur.

Geriatric Use

Clinical studies of PREGNYL® (chorionic gonadotropin for injection, USP) did not include subjects aged 65 and over.

The action of HCG is virtually identical to that of pituitary LH, although HCG appears to have a small degree of FSH activity as well. It stimulates production of gonadal steroid hormones by stimulating the interstitial cells (Leydig cells) of the testis to produce androgens and the corpus luteum of the ovary to produce progesterone.

Androgen stimulation in the male leads to the development of secondary sex characteristics and may stimulate testicular descent when no anatomical impediment to descent is present. This descent is usually reversible when HCG is discontinued. During the normal menstrual cycle, LH participates with FSH in the development and maturation of the normal ovarian follicle, and the midcycle LH surge triggers ovulation. HCG can substitute for LH in this function.

During a normal pregnancy, HCG secreted by the placenta maintains the corpus luteum after LH secretion decreases, supporting continued secretion of estrogen and progesterone and preventing menstruation. HCG HAS NO KNOWN EFFECT ON FAT MOBILIZATION, APPETITE OR SENSE OF HUNGER, OR BODY FAT DISTRIBUTION.

SAFETY INFORMATION

This instruction sheet complements, but does not replace instructions by your healthcare provider regarding the administration of Pregnyl®. It should be used after receiving instruction from your healthcare provider.

Pregnyl®, like all gonadotropins, is a potent substance capable of causing mild to severe side effects including Ovarian Hyperstimulation Syndrome (OHSS), with or without pulmonary or vascular complications.

Pregnyl® should be prescribed only by physicians who are experienced in infertility treatment. Physicians should advise their patients of treatment risks, including OHSS and multiple births.

Getting ready to use your

Pregnyl®
(chorionic gonadotropin for injection, USP)

Read all instructions before using your Pregnyl®.

You will need:

• a vial of Pregnyl® and a vial of Pregnyl® Solvent;
• one disposable 1-10cc syringe with reconstitution needle;
• one intramuscular injection needle;
• alcohol pads;
• a safety container to discard used supplies.

Wash your hands thoroughly with soap and water.

Recommended injection sites are the upper, outer quadrants of your buttocks.

Clean a two-inch space around your selected injection site with alcohol. Let dry at least one minute.

Remove the flip-caps, then clean the rubber stopper on each vial with alcohol. Let dry at least one minute.

IMPORTANT: Always use Pregnyl® as prescribed for you. Do not inject Pregnyl® until you have received instructions from your healthcare provider regarding the administration of Pregnyl®. Close care by your healthcare provider is very important.

READY: Prepare your Pregnyl®.

Carefully remove the cap from the reconstitution (mixing) needle on your disposable syringe.

Push the needle through the rubber stopper on top of your Pregnyl® vial, and slowly pull the plunger back, to withdraw the sterile air from the vial. Pull the needle out of the vial of Pregnyl®.

Push the needle through the rubber stopper of the Pregnyl® Solvent vial and depress the plunger to inject the air into the vial.

Turn your Pregnyl® Solvent vial and syringe upside down, and slowly pull the plunger back to the calibration mark appropriate for your prescription. Slowly pull the needle out while keeping the needle below the surface of the liquid.

Push the reconstitution needle through the rubber stopper of your Pregnyl® vial and inject the solvent into the vial.

Gently swirl your Pregnyl® vial until the solution is clear. DO NOT SHAKE.

IF the solution is not clear or has particles, DO NOT USE IT.

SET: Draw and check your Pregnyl®.

Gently pull the plunger back to draw the prescribed amount of Pregnyl® solution into your syringe.

Remove the reconstitution needle from the syringe and dispose of it immediately. Replace with a sterile, intramuscular injection needle.

Hold your syringe pointing up and gently tap the side, forcing air bubbles to the top.

Squeeze the plunger gently until all air is expelled and a drop of Pregnyl® appears at the needle tip.

INJECT: Inject your Pregnyl®.

The intramuscular injection of Pregnyl® should be done by your partner. Carefully follow your healthcare provider's instructions.

Shift your weight to the opposite leg and relax the muscle.

Stretch the skin at the site, and fully insert the needle at a 90-degree angle.

Start from about three inches away and push the needle in with a smooth, quick thrust, to cause the least discomfort.

Pull back gently on the plunger.

If the needle is correctly inserted, it will be difficult to pull the plunger back.

If you see blood in the syringe after you pull back gently on the plunger, pull out the needle, and apply pressure to the site with an alcohol pad. Replace with another sterile, intramuscular injection needle and repeat Steps 2 and 3.

Once the needle is inserted properly, slowly and steadily depress the plunger until the syringe is empty.

Pull the needle out quickly, and apply pressure with an alcohol pad. Maintaining pressure, gently massage the site.

Dispose of the needle and syringe as instructed by your healthcare provider. DO NOT REUSE.

You have completed your injection!

GONADOTROPINS, CHORIONIC - INTRAMUSCULAR

COMMON BRAND NAME(S): A.P.L., Chorex, Pregnyl, Profasi

USES: This medication is a hormone used in boys (before puberty) to cause the normal dropping of the testicles into the scrotum. It is also used in certain boys to help with normal sexual development. It works by causing the testes to release male sex hormones (e.g., testosterone).

This medication is also used in women to treat fertility problems. It is given after finishing another medication (menotropins) to cause the release of an egg (ovulation). It should not be used in women whose ovaries no longer make eggs properly (primary ovarian failure).

This medication has not been shown to be effective for weight loss and should not be used for this purpose.

HOW TO USE: This medication is given by injection into a muscle by a health care professional. Learn all preparation and usage instructions for this product. Before using, check this product visually for particles or discoloration. If either is present, do not use the liquid. If you have any questions about the use of this medication, consult your doctor or pharmacist.

The dosage is based on your age, weight, medical condition, and response to treatment.

For boys, use this medication usually 3 times a week or as directed by your doctor. For women, use this medication usually for 1 dose after finishing your menotropins treatment.

Use this medication regularly to get the most benefit from it. To help you remember, mark the day(s) on the calendar when you need to receive the medication.

SIDE EFFECTS: Headache, restlessness, tiredness, or pain at injection site may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Tell your doctor immediately if any of these unlikely but serious side effects occur: swelling of the ankles/feet, mental/mood changes (e.g., depression, irritability), signs of early puberty in boys (e.g., facial/pubic hair, voice change, acne), pain/swelling of the breast (in boys).

For women, when this medication is used with menotropins to treat infertility, also tell your doctor immediately if this unlikely but serious side effect occurs: abdominal pain/pressure/swelling.

For women, when this medication is used with menotropins to treat infertility, seek immediate medical attention if any of these rare but very serious side effects occur: chest pain, slurred speech, weakness on one side of the body, confusion.

A very serious allergic reaction to this drug is rare. However, seek immediate medical attention if you notice any symptoms of a serious allergic reaction, including: rash, itching, swelling, severe dizziness, trouble breathing, shortness of breath.

PRECAUTIONS: Before using this medication, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies.

This medication should not be used if you have certain medical conditions. Before using this medicine, consult your doctor or pharmacist if you have: early puberty (in boys), male sex hormone-dependant cancer (e.g., prostate cancer) in boys.

Before using this medication, tell your doctor or pharmacist your medical history, especially of: heart disease (e.g., congestive heart failure), kidney disease, seizures, asthma, migraine headaches.

A preservative (benzyl alcohol) that is found in the liquid used to mix this product can infrequently cause serious (sometimes fatal) problems if given in large amounts (more than 100 milligrams per kilogram daily) to an infant during the first months of life. The risk is greater with low-birth-weight infants. Symptoms include sudden gasping, low blood pressure, very slow heartbeat. If you notice any of these symptoms in your newborn, report them to the doctor immediately. If possible, a preservative-free product should be used when treating newborns.

This medication must not be used during pregnancy. If you become pregnant or think you may be pregnant, tell your doctor immediately.

When this medication is used in females with menotropins to treat infertility, it may cause multiple births (e.g., twins, triplets).

It is not known whether this drug passes into breast milk. Consult your doctor before breast-feeding.

DRUG INTERACTIONS: Your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them. Do not start, stop, or change the dosage of any medicine before checking with your doctor or pharmacist first.

Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription/herbal products you may use.

This medication may interfere with certain laboratory tests (including luteinizing hormone-LH/follicle stimulating hormone-FSH levels), possibly causing false test results. Make sure laboratory personnel and all your doctors know you use this drug.

This document does not contain all possible interactions. Therefore, before using this product, tell your doctor or pharmacist of all the products you use. Keep a list of all your medications with you, and share the list with your doctor and pharmacist.

OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. US residents can call the US National Poison Hotline at 1-800-222-1222. Canada residents can call a provincial poison control center.

NOTES: Do not share this medication with others.

MISSED DOSE: If you miss a dose, contact your doctor or pharmacist immediately to establish a new dosing schedule.

STORAGE: Store the vials of powder at room temperature between 59-86 degrees F (15-30 degrees C) away from light and moisture. Store the mixed medication in the refrigerator between 36-46 degrees F (2-8 degrees C). Write the date when the medication was mixed on the vial. The mixed medication must be used within 1 to 2 months, depending on the product. Check the product package for the storage time for your product. Keep all medicines away from children and pets.

Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company for more details about how to safely discard your product.
