What Is Fexofenadine and How Does It Work?

Fexofenadine is an antihistamine used to relieve allergy symptoms such as watery eyes, runny nose, itching eyes/nose, sneezing, hives, and body itching. It works by blocking a certain natural substance (histamine) that your body makes during an allergic reaction.

Fexofenadine is available under the following different brand names: Allegra, Allegra Allergy 12 Hour, Allegra Allergy 24 Hour, Children's Allegra Allergy, and Mucinex Allergy.

Dosages of Fexofenadine:

Dosage Forms and Strengths

Tablets

  • 30 mg
  • 60 mg
  • 180 mg

Oral suspension

  • 30 mg/mL

Oral disintegrating tablets

  • 30 mg

Dosage Considerations – Should be Given as Follows:

Seasonal Allergic Rhinitis/Chronic Idiopathic Hives (Urticaria)

  • Adult: 180 mg orally once/day or 60 mg orally twice daily

Seasonal Allergic Rhinitis

  • Children under 2 years: Use not recommended
  • Children 2-12 years: 30 mg orally twice daily
  • Children over 12 years: 60 mg orally twice daily OR 180 mg orally once/day
  • Allegra ODT
    • Children 6-12 years: 30 mg orally twice daily

Chronic Idiopathic Urticaria

  • Children under 6 months: Use not recommended
  • Children 6 months-2 years: 15 mg orally twice dailyv
  • Children 2-12 years: 30 mg orally twice daily
  • Children over 12 years: 60 mg orally twice daily OR 180 mg orally once/day
  • Allegra ODT
    • Children 6-12 years: 30 mg orally twice daily

Dosing Modifications

Renal impairment (CrCl less than 80 mL/min): 60 mg orally once/day initially

  • Children under 6 months: Safety and efficacy not established
  • Children 6 months to 2 years: 15 mg orally once/day, initially
  • Children 2-12 years: 30 mg orally once/day, initially
  • Children over 12 years: 60 mg orally once/day, initially

Administration

  • Allegra ODT: Allow to disintegrate on tongue, followed by swallowing with or without water; take on empty stomach; do not chew
  • Co-administration with grapefruit, apple, or orange juice reduces bioavailability of fexofenadine by inhibiting P-gp; separate administration by at least 4 hours

  • ombitasvir/paritaprevir/ritonavir
  • phenelzine
  • ponatinib
  • quinidine
  • sofosbuvir/velpatasvir
  • vemurafenib

What Are Warnings and Precautions for Fexofenadine?

Warnings

  • This medication contains fexofenadine. Do not take Allegra, Allegra Allergy 12 Hour, Allegra Allergy 24 Hour, Children's Allegra Allergy, or Mucinex Allergy if you are allergic to fexofenadine or any ingredients contained in this drug
  • Keep out of reach of children. In case of overdose, get medical help or contact a Poison Control Center immediately

Cautions

  • Allegra ODT contains phenylalanine
  • Severe kidney (renal) impairment
  • Co-administration with fruit juice may decrease efficacy

Pregnancy and Lactation

  • Use fexofenadine with caution during pregnancy if benefits outweigh risks
  • Animal studies show risk and human studies are not available or neither animal nor human studies were done
  • Fexofenadine's excretion in milk is unknown; use with caution if breastfeeding (The American Academy of Pediatrics [AAP] states fexofenadine is “compatible with nursing”)
